Sports physicals for middle and high school students are done in our office as full well-child check ups. During these physicals, our physicians will cover more preventive health counseling and do a more complete exam than is done during a physical at your high school or at an urgent care facility. Most insurance companies do cover this service in our office since it is performed as a well child/person exam.

If your middle or high school child is scheduled for a check-up and needs the sports physical form completed, please arrive with the second page of the sports physical form completed which covers health history. This form is available from your child’s school or may be downloaded from the Virginia High School League website www.vhsl.org. The physicians need to review the medical history part of this form with you before completing your child’s physical.

If your child had immunizations done in another physician’s office or clinic or at the health department, please bring those records with you to your appointment so that school or camp forms can be fully completed by our staff during your visit. Do not assume that records have been received from your previous physician.

If a medication form is required for school, please bring it to your child’s well check appointment.
